Patient-Centered Medical Home (PCMH) Model Virtual Training

As a part of the TECPHI Opioid Supplement approach to improving public health planning and infrastructure, the USET Office of Tribal Public Health (OTPH) is offering a Patient-Centered Medical Home Model training to USET member Tribal Nations, USET staff, and NAO IHS staff. Patient-Centered Medical Home (PCMH) model is an emerging healthcare model. According to the CDC, the patient-centered medical home (PCMH) model is an approach to delivering high quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. The PCMH model has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.
When: December 3 at 10:00am – 2:00 pm Central/11:00 am – 3:00 pm Eastern (include a 30-minute lunch break)
Who Should Attend: Tribal Nation clinic staff, IHS clinic support staff, and USET OTPH staff
Learning Objectives:
- Understand the function of a PCMH.
- Gain knowledge of how to create a more efficient and culturally-centered approach to whole-person health care delivery.
- Develop skills for patient intake and tracking with integrative care through a PCMH.
